Custom HIPPA Form Module Field Glossary

Text – This is a standard text field for questions like ‘Name’, ‘Company’ etc.

Hidden Text – This is a text field where the answer is obscured as it is written. Use it for things like personal id numbers or passwords

Text Area – This is a longer text box for questions like ‘Message’ or ‘Explanation’.

Number – This is a field where the user can only add numerical answers

Range – This is a field where the user can choose between a minimum or maximum value (1-10, etc.) You can choose the size of the steps between those two values

Email –  This is a field where the user can only add an email address

URL – This is a field where the user can only add a valid URL

Select – This is a field where the user can select an item from a drop down. Note that you can add items to the dropdown by clicking the ‘Add Option’ button that appears when you select this field type.

Checkbox – This is a field where the user can choose one or more options by clicking a check box. Note that you can add checkbox items by clicking the ‘Add Option’ button that appears when you select this field type.

Radio Button – This is a field where the user can choose ONE option by clicking a radio button Note that you can add radio button items by clicking the ‘Add Option’ button that appears when you select this field type.

Yes/No Switch – This is a field where the user can toggle a switch to agree or disagree with a question.

 Message – This field allows you to add additional instructions or explanation such as when breaking up sections of a long form or grouping questions by subject matter.

Date Time – This field allows the user to specify a date AND time.

Date – This field allows the user to specify just the date.

Time – This field allows the user to specify just the time.
